Breeding Checklist & Forms
Thank you for your interest in our breeding program. Please review the following Breeder's Checklist and referenced forms below. Don't hesitate to Contact Us if you have any questions.
Breeder's Checklist
Prior to Breeding:
1. Review our Breeding Policies
2. Complete, sign and return the Sure Hit Breeding Contract
3. For frozen semen: complete, sign and return the Frozen Semen Contract
4. Sign and return the Breeding Soundness Report (must be completed and signed by your veterinarian)
5. Make full payment of breeding, collection and booking fees
Ordering Semen:
1. Notify us at 770-569-9555 when the mare ENTERS the heat you would like to breed. When you call, please provide the following information:
a) Your name and the mare owner's name
b) Contact phone number
c) Stallion's name
d) Mare's full name
e) Desired collection day
f) Desired shipping method (Equitainer or Disposable)
g) Shipping address
h) Destination airport, if applicable
i) Credit card number to keep on file for payment of shipping fees
2. Make required payments for chosen semen shipping method (Equitainer or Disposable)
3. Complete and return/fax the Semen Shipping Information Form (coming soon)
4. Call with anticipated breeding date
After Breeding:
1. Return Equitainer (if used) within 7 days
2. Call 770-569-9555 to notify us of pregnancy status
3. Complete and return Mare Breeding Record after conception is confirmed
